Helping your patients to move more and live well…

At Be Well, we work with healthcare professionals across Wigan Borough to support patients to improve their health and wellbeing through physical activity, healthy living support, and community-based services.

Our programmes support people living with long-term health conditions, disabilities, and those looking to improve their wellbeing through accessible activities, advice and guidance, and behaviour change support.

The benefits of movement…

Movement is a powerful tool in prevention and treatment of many conditions, and healthcare professionals play a vital role in encouraging people to become more active.

Evidence shows that even a brief conversation about physical activity can help improve health outcomes and reduce the risk of many long-term conditions.

Benefits of increased physical activity include:

  • Reduced risk of cardiovascular disease and type 2 diabetes
  • Improved mental health and wellbeing
  • Reduced risk of falls and frailty
  • Improved outcomes for many long-term conditions
  • Better quality of life and independence

Physical inactivity is a major contributor to preventable disease, yet many patients would become more active if encouraged by a healthcare professional.

Ways we can help

Our friendly, experienced and knowledgeable team can support your patients with many aspects of their health and wellbeing. We offer:

  • A full range of physical activities (including many free and low-cost options)
  • Support with being active with long-term health conditions, disabilities or mobility issues
  • Support with healthy weight and nutrition
  • Stop smoking services
  • Social prescribing services (community link workers)
  • Support in the workplace (WorkWell)

Patients can self-refer, or you can make a referral on their behalf.

Embedding movement in healthcare

The Royal College of General Practitioners’ Active Practice Charter, developed with Sport England, supports GP practices to integrate physical activity into routine care.

The charter encourages practices to:

  • Increase physical activity in staff and patients
  • Reduce sedentary behaviour
  • Partner with local physical activity providers
  • Promote physical activity in healthcare settings

Practices that implement these changes can achieve Active Practice status and demonstrate their commitment to preventative healthcare.

Resources for healthcare professionals

Moving Medicine

Moving Medicine is an evidence-based digital resource designed to help clinicians integrate physical activity advice into everyday consultations.

It provides condition-specific physical activity guidance; 1-minute, 5-minute, and extended consultation guides; behaviour change tools; and patient resources and evidence summaries. These tools enable clinicians to deliver brief interventions that encourage safe and effective movement.

RCGP Physical Activity Toolkit

The Royal College of General Practitioners’ Physical Activity Toolkit, developed with Sport England, supports primary care teams to embed physical activity advice into consultations.

The toolkit includes evidence-based guidance, patient communication resources, behaviour change tools, waiting room and digital assets, signposting to local activity opportunities. These resources help clinicians discuss physical activity effectively within short consultations.

Training opportunities

Healthcare professionals can access training to increase their confidence when it comes to discussing physical activity with patients.

The Physical Activity Clinical Champions programme provides peer-led training to support health and care professionals in integrating physical activity into routine care.

The programme helps clinicians:

  • Improve knowledge of physical activity benefits
  • Deliver brief interventions in consultations
  • Support behaviour change conversations
  • Promote preventative healthcare approaches
